DS-82 is a passport renewal application that must be filled out before the expiration date on your document. This is a form for the US passport renewal only.
This form does require numerous attached documents. If you want to renew the passport due to the expiration date, add the old one to it. You also have to pay fees and attach the photo.

The application form DS-82 Application for Passport Renewal is used when renewing a United States passport that is expired or near the expiration date for no more than five (5) years beyond the expiration date on the passport.

What is Needed to Use the DS-82 Passport Renewal Form?
- Two Identical Passport Photos: Include two identical professionally taken passport photos. You can obtain your passport photo at FedEx Kinko’s, Post Office, Walgreens, and CVS. These are a few places, amongst others. Head coverings are acceptable for religious purposes with proper documentation and prescription eyeglasses.
- Expired Passport Book: Submit your expired or soon-to-be Passport. Passports must be in good condition. The passport will not be accepted if it is damaged. Make sure your passport was issued within the last 15 years.
- Government Fee: A check or money order made payable to the “US Department of State” for $170. If you have applied for the passport card, the check or money order will total $200.
- Authorization Letter: Print one copy of the authorization letter. This allows us to act as a courier on your behalf. Please only check off the first two boxes on the authorization letter.
- Travel Itinerary: Print one (1) copy of your travel itinerary from an airline or travel agency. Ensure that the travel date on your itinerary matches the date on your application. If you have questions regarding this step, please call 877-910-7277.
