What is a Logic Model?

A logic model is a visual representation that maps out the relationship between the resources you invest, the activities you plan to undertake, and the outcomes you expect to achieve.

It is used to clarify the theory of change behind a program or project, showing how each part connects to the next to drive impact. Logic models are commonly used in program planning, evaluation, and communication with stakeholders to provide a clear and structured overview of how a program operates and its intended outcomes.

How to Use This Template:

  1. Inputs: Start by listing all the resources you’ll need to operate the program effectively.

  2. Activities: Define what actions will take place to implement the program.

  3. Outputs: Identify measurable products of those activities.

  4. Outcomes: Specify the expected short-term, medium-term, and long-term results.

  5. Impact: Describe the overarching, long-term change or difference the program will contribute to.

This structure helps to ensure that all program activities are logically connected to the desired outcomes and impact.
